If you are experiencing any technical issues, such as being unable to register or complete online trainings, print your completion certificate, or sign up for the Provider Orientation, please email Nicole.Garcia@ucsf.edu with the subject line "FPACT LMS Tech Issue".

Include as much of the following information as you can (it's ok if you don't have it all - this will help us troubleshoot): 

  • First and last name
  • Name of the training
  • Description of the problem (e.g., unable to print completion certificate)
  • Link to the page where you are having trouble
  • Screenshot of any error messages (if available)
